Official Form B201, also known as the Notice to Individual Consumer Debtor Under §342 of the Bankruptcy Code, provides important information about the various chapters of bankruptcy relief. 

All debtors are required to read this Notice before filing for bankruptcy relief.  Debtors must also file a signed Notice with the Bankruptcy Court swearing under penalty of perjury that they have read and understood the contents. 

This Notice should be filed with the Bankruptcy Court at the time of the original filing.  Local Rule 1007 requires the debtor to file a completed Notice within fourteen (14) days of the original filing.
